
Request Form for American Sign Language (ASL) Interpreter
The form is for any CSC-Hosted training, meeting, or event.
Submission:
Submit a separate form for each individual interpreter request.
Submission Time Frame:
Requests must be submitted at least two weeks before the scheduled training, meeting, or event. Requests will be reviewed within 48-72 hours.
Virtual Meetings & Training (Zoom & Teams):
Individuals needing an interpreter via Zoom or Teams must already be registered for the training, meeting, or event and must have received the virtual link before submitting a request.
Cancellations:
Cancellation requests must be made 72 hours in advance to the CSC ADA Manager via email
